HIPAA Notice | Effective Date: April 3, 2026

This notice describes how medical/psychological information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

1. Our Duty to Protect Your Information

We are required by law to maintain the privacy of your protected health information (PHI), provide you with this notice, and follow the terms of this notice. PHI includes information about your health condition, the care we provide, and payment for that care.

2. How We May Use and Disclose Your PHI

  • Payment: We may use your PHI to bill and collect payment from you, your insurance, or a third party.

  • Abuse or Neglect: We may disclose PHI to appropriate authorities if we reasonably believe you are a victim of abuse, neglect, or domestic violence.

3. Uses Requiring Your Written Authorization
