Notice of Privacy Practices

This notice describes how medical/psychological information about you may be used and disclosed and how you can get access to this information. Please review it carefully.
1. Our Duty to Protect Your Information
We are required by law to maintain the privacy of your protected health information (PHI), provide you with this notice, and follow the terms of this notice. PHI includes information about your health condition, the care we provide, and payment for that care.
2. How We May Use and Disclose Your PHI
- Treatment: We may use your PHI to provide, coordinate, or manage your psychological care.
- Payment: We may use your PHI to bill and collect payment from you, your insurance, or a third party.
- Healthcare Operations: We may use your PHI for quality assurance, training, licensing, and accreditation.
- As Required by Law: We may disclose PHI when required by federal, state, or local law.
- Abuse or Neglect: We may disclose PHI to appropriate authorities if we reasonably believe you are a victim of abuse, neglect, or domestic violence.
- Serious Threat to Health or Safety: We may disclose PHI to prevent a serious threat to your health or safety or that of others.
3. Uses Requiring Your Written Authorization
We will obtain your written authorization before using or disclosing your PHI for purposes not described in this notice, including marketing purposes. You may revoke any authorization in writing at any time.
4. Your Rights Regarding Your PHI
- Right to access and obtain a copy of your PHI
- Right to request amendments to your PHI
- Right to an accounting of disclosures
- Right to request restrictions on certain uses
- Right to request confidential communications
- Right to a paper copy of this notice
- Right to file a complaint if you believe your privacy rights have been violated
